Why not...: Btcinstant: $8 fee + only one transaction. Bitcurex: Card costs $30USD, and adding money to it does not get a very good exchange rate. XMLGold: 20$USD card (this is okish actually) + monthly fees + ~7% exchange rate from BTC to USD.
