Assuming Bitcoin-QT, just click "Add Recipient" in the "Send coins" tab.

Beats me. Unsure why it'd have a limit since default minimum bid/fee increases with size of transaction. Definitely more than three, though.

Bitcoin is not meant for sending tiny amounts to a lot of people.
