I'm bringing up this topic because of some of the horror stories I've heard from some investors and early adoptors looking to say cash out 0.1-o.5BTC or higher. For example sending your BTC to coinbase only to see it deposited in a hackers account without ever having made it to the exchange. Does anyone of any stories like this?

I'm going of forum posts and some rumors, but considering the ammounts I mentioned that's weeks or months of work for some people.

Strategies for avoiding this and where to go would be greatly appreciated.

Well, there are a few things you should do if you cash out your BTC.

1. Use an exchange you can trust (f.e. kraken.com, bitstamp.net), also make sure you haven´t landed on a phishing site (always enter the url yourself and don´t use links from emails), make sure there is https:// in your url
2. When sending your bitcoins to the exchange make sure you send to the right address (double check before sending)
3. Make sure you pay your taxes if you have to pay any (f.e. I don´t have to in my country)

So Far, I have been using two exchanges for more than a year, Bittrex and Poloniex and never been encountered or lost coins with them, although admittedly they do have some support problems as sometimes took days and need further follow ups to twitter before doing an action although the problem still solved, what I been always doing is that always enabled 2FA, some sort of added security.

I never sale bitcoins because bitcoin is gold. If you want to sell then you must keep some usefull info that never sell to any exchange that uses adds. Use bitstamp because it gives you better price then other exchanges. Also dont sell to anyone which offers you more then the actual price or use escrow with those users.

Never encountered any problems? Where were you when there was a DDoS attack at Polo?
I lost my altcoins (not BTC because I didn't have any in my account at that time) and many complained they lost their Bitcoins too, that's a different thing that they managed to get out of the attack without any harm to the capital of their users and credited our accounts back in 48 - 72 hours.
I think that if OP manages to be a bit more careful and feasibly does his transactions to the correct exchange address and uses it, then there shouldn't be a problem of a hack. Use Localbitcoins if you feel so scared about going for online deals because that allows you to go for cash transactions too and you may find people who are already in your trust circle.

it is never the exchanges fault. so your question is kind of strange asking which exchange is best.
it usually is the fault of the user where they put a false address in their withdrawal or have some kind of malware on their computer that causes things like this or even have malicious software instead of a legit bitcoin wallet!

it depends on what you are trying to avoid but generally i can say:
keep your computer clean!

use a trusted exchanger , usually exchangers like that when the registration ask for personal details in detail to know the member.
Example www.bitcoin.co.id , www.luno.com , www.kraken.com don't use exchanger services that are too easy at registration.
always wary of exchangers that do not display their cloud storage

I am currently using a couple of exchanges although the exchange site I am mostly using or exchanging my altcoins to bitcoin would be Bittrex and Poloniex. I never had any issues with these two sites especially with their withdrawal of bitcoins as well.

Hmm. For me I use a lot of exchanger in my transactions because we all know that there are different kind of exchangers and it depends on it's value of exchange. But my most used exchanger are Bittrex because as I can see based on my previous transactions are the values in bittrex are quite good and very convinient in their services.

I always use Poloniex in my exchange transactions because besied of being the most popular I haven't experience any casualties like scams or other delay transactions in poloniex and the price there was good although sometimes it has a high fee. I also there in the Poloniex of being weill updated every moment.

I have never experienced anything like your story, I've been using the market for a long time like coinexchange, poloniex, bittrex but to date I've never had a problem, I use 2FA for extra security, your story is very scary for this, my advice you should be more careful in taking care of bitcoin.

easy to avoid that does not happen, you should look for accurate information about the exchangers before making a deposit or you can choose the exchanger that already have a trust like kraken, okcoin, etc.

I think that kraken.com & bitstamp.net are the best for selling btc.

Since I have started I do exchange my bitcoin whenever I sells indeed of situation to get the fiat. I tried all the times in the localbitcoins site but few months before they have added a deposit fees to add the bitcoin to the exchange site.

You will be able to get the more value than you see in the Google price index. Traders will be able to send to bank account and other online wallet options with the secured automted escrow.

I always uses cryptopia for selling bitcoin . I have used a lot of time but never been cheated . Actually this is one of the best exchangers . While exchanging bitcoin  first thing you should check that are you in the right url . If you are in right website you will never lost your money . And if you wrongly entered in phising site you may lost all of your money . So always check that .

So far,i have used only two exchanges,Poloniex and Bittrex.

Poloniex is considered to be the most legit exchange.It has high trade volumes.Also,the withdrawal fee is very low,just 0.0001 BTC.It remains unchanged.

Bittrex is also a legit exchange.It has user friendly interface.Here,every time we trade,trading fee is clearly mentioned for every trade and so nothing remains suspicious.

But if you use newer exchanges without researching,either you may be charged very high transaction fee or your bitcoins might get compromised totally.

Bittrex, Poloniex and cryptopia is the best exchanger to me..I always exchange coin in this site. legit exchange, low fee,easily used.

Be careful because you may be foolish and you can not recover your bitcoins.
If you sell a lot of exchangers who come with that and you will find it hard to find the truth and you do not like it. Local Bitcoins here sell you to a person but you should not trust instantly because you might be robbed. Better recognize and preferably if you get her IDs to be sure...

Can you say what is that country? Where do you live if you dont mind sharing that info? Are you sure that you dont have to pay taxes when you cash out?? Maybe bitcoin itself is not taxable but when you sell your bitcoin for fiat that fiat becomes taxable.

Follow these steps and you won't lose a satoshi
1.Check exchange reputation if it is reliable.
2.Double check the deposit bitcoin address you are sending bitcoins too.
3.Only send bitcoins to sell.Don't use exchange as a wallet.
4.Use strong password with 2FA on that exchange.
5.Sell and withdraw fiat.
OR
Use localbitcoins and sell for cash instead.(Safest option) :)
