in the link bar of your browser you can see your user id number when clicking on your own name in bitcointalk. that is first field you need to fill in. before completing the form go to -Forum Profile Information- here on bitcointalk (click your own username to see the option on the left). fill your ETH address in the -Location:- (right above -gender- field and under -birthday- The other fields to use in postoken are yours to choose (email and pw and the recaptcha) thats all. good luck.
