I think http://bitcoinexchange.cc/ will pay you dollars for bitcoins via Paypal, but not the other way arround.

I purchased some bitcoins to them in the past, and they were very trustworthy.

You can use Virwox www.virwox.com (http://www.virwox.com).
But I think you will need a Second Life account.

This is done by many individuals on the #bitcoin-otc marketplace
  - http://www.bitcoin-otc.com

Because you are sending an irrevocable payment, know that you should consider the other party's gpg trust rating.

ClearCoin escrow sadly is no longer accepting new listings.  There are other escrows now but because they are new they  too should receive a dose of caution.

Also, there is an online service which will cashout to PayPal:

Why don't you just withdraw your funds on your bank account, then login paypal and add funds to your pp account?

Why would you want to pay triple fees if not just to hide the origins of the cash?
That's none of my business though.

The easiest way is Mt. Gox -> Bank. Doesn't take more than a few business days.

Yeah I have paypal, if you send coins I send you money to Payapal.  Seems to be the best method, I know bitmarket.eu you can use paypal to paypal via members just like if you want to send me 20 coins I can send you money to paypal and there you do, bitcoins to paypal.  Or you sell your bitcoins for money to paypal.
