Go to https://imgur.com/upload > Choose your image and upload it > Hover over the uploaded image there > Go over the drop down button and choose "Get share links" > There will be 4 different codes by default, choose the one that has been labeled "BBCode (Forums)" > copy that code over here and everything should work properly. It's also happen to me when I upload old hosted image or it is dead image link. Most likely you're using the wrong code for forum.
