See Giftocard, they issue a Mastercard VCC and accept btc as payment. I have worked with them for 5-6 months and never had any problem. Neteller is another option you can choose from.

It's accepted everywhere Mastercard is accepted. I received my VCC a few hours after the payment. You can use any address you want on it, and its limit is 2500 USD (as far as I remember). The card is valid for 1 year.
