In summary, unless you pay the 5% for manual processing of your withdraw, you cannot withdraw CAD/USD from MtGox right now (you haven't been able to since june). BitStamp and BitFinex are good exchanges that support CAD/USD withdraw. If you're in the US, check out CampBx, they support ACH withdraw directly to your bank account. They are a good company too. CoinBase also works in the USA, also a good company. If you're in canada, CaVirtex is available to Canadian citizens. I wouldn't know about withdrawing in USD, I've never done that there. I like Coinbase for general purposes.
