I've had this problem happen when atmosphere temperature is too cold or too hot. It is indeed PSU problem (or at least that was my conclusion at the time).

A simple turning off of machine for 2-3 mins and then plugging it back in would almost always fix the problem.

If that does not fix it, try unplugging chain 0 and trying with other two chains.

Seems like a PSU issue to me, the best way to troubleshoot this issue now would be disconnecting chain 0 and chain 1, leave chain 2 only, allow the miner to run for 30 mins and then post the new kernel log, also, you might want to give the PSU a good dusting, it could be acting funny because of the high temperature.

If that doesn't fix it, try custom firmware like Asic.to see if that gives you any different resutls.
