There is no requirements besides your address and to test the product/game.

We will monitor your address and after we will notice you played the game we will send you again more tokens + bonus .

This step will be repeated 4 times.So in the 4rth time we will send you token you might stop testing and keep the tokens for yourself.

The proposed token which you planned to send to the participants, are they to be test net tokens which can only be used on your platform, or they are going to be general tokens which can actually be withdrawn by the user.

Another thing I would have advice you make the application open on this board so that anyone how is interested on testing the platform can drop an application and suggestions here on your thread and possibly some reviews too.  

According to the form, this requires a matic address, not a bitcoin one.  You are in the wrong section.  All offers that are published in this section are paid in part or in full in bitcoins.  The altcoin board is convenient for posting your offer. 
According to your posting history, this is not the first time you have posted an offer to ask for testers. Can you be precise about the value of tokens you aim to offer as reward? tell about much budget you have for testing stage?

As I see you are a new bie here so I would welcome you onboard. I assume you are the sole representative of your casino here and this I must say that this thread is likely assumed to be your official announcement thread but on the long run while doing your beta test I would advise you upgrade to a copper member so you could upload the graphics outlook of your product for first hand view by members because that would show us how far you have gone with your development and other things involved in your game product.

Review can be made here in line with your product development from members while the beta phase is ongoing.  It is nice you identified with this platform. Be rest assured that you would get the best, honest and genuine review from members here.

If you plan launching soonest after beta test, you can make plans for promotion and publicity as there are a million members here ready to give your game a good trial but you must also consider running a signature campaign if you can which would be managed by a good manager with good reputation and audience here.
