Casascius (Mike Cladwell) stopped production for a while now. Usually what happens is when you buy a Casascius coin from Mike, you send him the BTC for the cost of the coin + BTC value of the coin, as denominated by different version of the coin you're getting. He ships you the coins unloaded, you confirm receipt of coins and he then funds them (with the BTC you originally provided). So yes, they sort of do come preloaded.
