Obviously on the fly. What OP probably does is generate some private keys on the user's end through Javascript based on which page you are; if the address is not empty, sends the private key to his server. Where he can send the coins to his own wallet. Basically, everybody that tries this is using his own hardware power to brute-force private keys for OP. Free work.

I'm just confused because I saw a video on youtube and it looks like it needs to devices the one is for offline(holds private keys) and another device is for watch only wallet. You can do that. Or you can just use the hot wallet (which he's using and I've used many years ago).

If you use Chrome and Google is your main search engine, pretty sure Google will have an excellent picture of who you are as a person. If you care about that, then could be a good idea to switch Browsers, or invest in some other blocking tools like a few users have mentioned.
DuckDuckGo over Google as your search engine is a must. I'm not sure if I should trust Brave, so I keep using Firefox. But being based on Chromium (which AFAIK is "open-source Chrome") doesn't make it less-privacy oriented? I don't know how that works for sure.

You can also create a Segwit address that starts with "3", which are supported everywhere and offer lower fees. 1. Go to iancoleman's BIP39 seed generator[1] (optional - download the source code and run offline for better security); 2. Select 12 words and generate a new seed; 3. SAVE THIS SEED AND DON'T LOSE IT! You can't see it again in Electrum and without the seed you can't restore your wallet in the future; 4. Download Electrum; 5. While creating the wallet, select: Standard Wallet -> I already have a seed -> Paste the seed from step 2. 6. Click Options -> check "BIP39 seed" -> Next. 7. In the Derivation part, paste m/49'/0'/0'/0 and click next. 8. Now you have an Segwit wallet with addresses starting with 3.

Yes. Some coins aren't fully supported on the Ledger Live. This means that you can't Send/Receive coins just with the Ledger Live software. You'll have to download a wallet for the coin that supports Ledger (NEON wallet in the case of NEO). Unfortunately not many coins. The new Ledger Nano X supports 100 coins app installed at the same time. While the Ledger Nano S only supports 3~5 (depending on which apps you want). But you can always uninstall the app and install a different one that no coins will be lost. Example: you have the BTC and ETH app installed. But you want to manage NEO; you uninstall the ETH app, install the NEO and do what you want. Later, you can uninstall the NEO and install the ETH again. Every coin will be safe, and you just have to install the app again to see and spend them.

iOS doesn’t give you any liberty to check the files and rooting it (jailbreak) is a pin in the ass and doesn’t work on the latest versions.
For Android (the app isn’t out yet) you will probably be able to see and edit them if your phone is rooted.

Unfortunately i am not at home currently, so can't test it. Did it happen after an update for something ? Or did it randomly occur ? I will definitely test signing a transaction via nano s + electrum when at home and report the result here.
This has been happening for a couple of months. I’m not sure what caused it, but I already accepted the delay and have been using the Ledger + Electrum for some time (the Ledger Live doesn’t have a ‘send max’ button). I’ll probably end up reinstalling my OS when I have the time.
