Yes, this is the main thing to see whether the exchange is trusted enough or not. Many exchanges did hold the user's big amount of withdrawal, even I heard Hitbtc did it many times when the user tried to withdraw the fund. But still, top exchanges should have reduced the fee long ago to make traders comfortable to withdraw their money, as the did now, so I think there will be no issue about withdrawing funds. It's better late than never.

No, mate, it did not start from Binance, rather OKex was first. After oKex reduced the fee, Kucoin, and Binance followed them. I think many exchange already did this to stay in the competition.
