Trying to get more Bitcoins so decided to try the BitBox Game I clicked on the box and I get a message saying- 'Excellent- You Have Found Tokens Inside! Complete Simple Verification Process to Claim Your Reward' then it says :- ' Oops! We Could Not Locate Your Form' how am I supposed to Play this game if it says it can't Locate my form??, what form??

Tried to play BitBox Game and it says I can open the boxes that someone left behind so I click on each box and get a message  saying :- Excellent- You Have Found Tokens Inside, Complete Simple Verification Process To Claim Your Reward and under that message it says "Oops! we could not locate your form" so how can I Complete the Verification and Claim the Rewards??
