It seems that your blkindex.dat has been corrupted. Did you just change your wallet.dat without enabling "Detach databases at shutdown" on Bitcoin > Options? Worst case, you'll have to delete everything except for the wallet.dat on your Bitcoin directory (not the one on Programs), then redownload again the whole transaction block chain or torrent it.
