When someone looks up a bitcoin shortname for example http:bit.co.in/metart what's the best string to use to generate the QR code?

Should it be just the plain payment address or should I append a "bitcoin:" before the address?

Then I've also seen people do bitcoin://...?amount=xxx or something like that.

Which makes most sense in case anyone knows?
