best exchange for BTC/USD, USD/EUR, BTC/ETH, USD/ETH?

Does anyone know a good trustworthy exchange that has all of the following: BTC/USD, USD/EUR, BTC/ETH, USD/ETH.  I used Cryptsy before and now I need a new one but I want to make sure it's trustworthy.

I don't think there'll be any exchanges that do fiat to fiat. Bitfinex is adding ETH soon for USD. Kraken does it for EUR.
