help filling out mtgox transfer of claim

What do I have to fill on page 5 in "Transferor"?
Below comes the "Transferee", that me as I understand?

How about using google translate and translating them to your own language, stop being lazy, you should have already done that by now, words you don't understand in english, you either go to google translate either go to google website and type that word with the word "definition" after that. Maybe this cannot solve your problem 100% but I am sure this helps at least 85% to solve the problem you are having.
