Help with ANACS form

I'm new to the forum so apologies if I've overlooked previous answers to this

I have a 2011 series 1 "misprint" Casascius coin that I would like to get graded via ANACS.

Is there anywhere a writeup has been done to filling out the form?

Do you typically get the "professional conservation"? I'm concerned about the "caustic chemicals" and the hologram...

What about the other options like Verification and Research? Seems to me like they shouldn't be necessary and take additional time.

How do you insure the coins? For the bitcoin value? Will the USPS recognize this as a legitimate claim should anything happen?

For the trip to ANACS you can send USPS Registered and Insured if you are US based.  For the return trip they will only be insured for $100 per coin as that is the ANACS limit for Crypto coins. For the bitcoin value? Will the USPS recognize this as a legitimate claim should anything happen?

There are quite a few threads similar to this.
As far as I can recall either monkeynuts or Blazed posted a few tips regarding the grading process.

Edit: Seems like the link in the post isn't working. Your best bet would be to message one of the users who frequently get coins graded with ANACs.

1. Yes
2. I always leave mint mark blank
3. Owners value can only be up to 100 USD for Physical Cryptocurrency (See bottom left of form where the price is $19 per coin etc).
4. Variety - This is where you put what you want to show on the slab. In your case, I would put CASASCIUS 1 BTC BRASS "CASACIUS IN HOLOGRAM" (XXXXXX) (Where xxxx == firstbits on hologram).

Google for a picture of 2011 error ANACS coin and you'll see what I mean. Sometimes they won't put the first bits down if you don't include them etc etc.
