imn using stock bios and all and the miner sees the 4th card but it never goes through all the steps in the begining like theres to lonbg a delay so it isnt read. however i can see the cards fine in device manager and in claymore.

need help getting the 4th card running
asrock h170 pro4s 7600k
1000watt EVGA psu 10yr warranty
gpu's rx480 and 580 x2

Please clarify, as we are willing to help, we need to understand what your 4th cards problem? is it in the subject, if yes, have you tried replacing your riser?

well i got all the cards running and they run fine if they are all on one PSU, however it is to much for one psu as it will shutdown cause overheat very good quality psu.

so i bought a add2psu device and a second psu. the riser is fine as i checked them out just that 4x 580 even with modified low power is just to much for a 750watt psu so i added a psu hooked the one card and riser to it and well that psu turns on the system sees the 4th card but when the miner starts it reads all zeros for that one card.

the add2psu is a device i bought that was recommended but i just cant seem to figure out why it (second psu) wont power the card to mine  in the miner. also i plan on adding one more gpu down the rd if all goes well

It sounds like it must be a riser problem (faulty riser/ bad usb cable  and that it is not the psu that is in fault.  You could also try and change pcie slot for this particular card and see if  you see any changes and report back. also how  is your setup on the dual psu side,  example:  all risers powered from psu one , what type of risers are you using ?
