Really want to hear from the community some of the things they've personally done to ensure themselves and other of the authenticity and trust of a Project and the types of checklist style requirements needed to be considered not just a authentic project but also have potential, and maybe hear some of the stories of things going wrong

If you try to look around in every corner of this forum, you could find a lot of points and factors on how to check and ensure the legitimacy of an ICO but not all of them are really into the best cetera of knowing the real or fake of everything.
For me, in my own personal opinion as this one is very highly effective in my checklists on how to find a good ICOs.
1. Good ICOs have a very strong social communities.
    - no matter how beautiful their web sites, roadmap, people behind etc. Their strength is in their social communities.
    - Good ICOs hires good bounty campaign managers. There are failed ICOs because their bounty managers are newbies or Jr. Members.
There are some more but these two factors works for me.

First just do your own due diligence, after you are personally satisfied with the project and the team behind, look out what the mentors, experts and the community is saying about this project, if wleverything looks good and positive the project is good and you can invest.
