Circle - Free bitcoin withdrawals but I don't know if friendly for gambling or mixer
Coinbase - Free bitcoin withdrawals (if amount larger than X) and NOT friendly for gambling or mixer

What are other providers?

For lots of micro-payments, you may prefer the exchange or service to cover your fees. There are zillions of services with the fees; but which are the ones without?
