What I need?

One simple and functional Iframe or Bitcoin payment page module (with qr code) to use in php website

The page will generate a new payment address for each new user (cookie will be used)
The server wallet solution will be also provided or the page will use blockchain wallet (cold storage will be a good option)
The user will choose payment value in BTC. Minimum payment is 0.019 BTC, maximum is 0.999 BTC
The user must complete on the payment page his nickname/username (this is not optional)

When payment is confirmed on another page the website will publish the list of confirmed payments with details: date | user X | paid Y BTC  | link to the transaction on blockchain.info. Also an email will be sent to an email address to inform about the payment received.
