Honestly I'm glad I don't have to deal with an authenticator on certain cites as it's frustrating if you've lost a phone and can't reconnect to that app. I think features that send a code to a phone number as a back up method is the way to go.

No matter how much you are securing/protecting a certain exchange against hackers, there is nothing you can do against the owners being the thieves here. If they want to run with millions worth of coins, then they'll surely do so.

Yes, we can do our part as users to implement all the best security layers we can but if we don't hold the private keys we have no bitcoins.
Third-party exchanges will be always dangerous: there's no 100% security for them when they have to deal with hot wallets.
