Yes, once It's done there is no coming back and that's why you should make sure you're trading with trustworthy services/people.

Depends:
As soon as your transaction is confirmed, it's irreversible, especially if it has 3+ confirmations. As long as a broadcasted transaction is not confirmed, there are technical ways of voiding these transactions (altough these tricks are not foolproof).
