I will need Name, Address, City, State and Optionally Zip and Phone for the lookup .
This an information only service and I am not responsible for your movements. Please do not ask for anything other than the service offered.
Mother's Maiden Name is available at an additional charge due to the complexities of obtaining that information it is not included in the standard lookup.
Price is $15 per lookup.
