- Your government issued photo ID (passport, permanent residence card or driver’s license) and
- A scan of either your monthly utility bill (power, phone, TV, gas, water, etc.) or a certificate of residency issued by your local government.

Don't fall for it. I was about to email them the documents.

You shouldn't still be using the same email address as you did before MtGox was hacked.

No, I don't. Knowing which email address you use opens several more ways to get into the account. I got the same email on the old account, but I knew it had to be phishing because it's not the one I use for MtGox anymore.
