I'm using Coinbase and Circle.  I was using Buttercoin but they're done.  Are there any others as reliable as these?
Whoever mines the block which ends up containing your transaction will get its fee.

You can check btc-e.com and melotic.com and bleutrade.com are reliable and well known sites which deals btc-usd directly.I made few transaction on all these site and worked perfect for me .

For nice volume you could use bitfinex or bitstamp yes bitstamp was hacked but they handled it like pro's and know body lost out so in my book they can be trusted. Take a look at BitcoinWisdom and make your choice from there Smiley

these are nice exchanges for btc/usd with good trading volume. so you can try these exchanges.

I should have said I'd like to be able to do a bank account transfer like with Coinbase and Circle.  Can Bitstamp, Bitfinex, or OKCoin do that?

Bitfinex.com - has the largest and trustable volume in BTC/USD
Bitstamp.net - also trustable
Btc-e.com - USD withdrawal fee is expensive
Okcoin.com - I don't think their trade volune is real

than you can try bit-x link in my signature with much active support and legit, now you can take withdraw using their master card service from any atm machine.

I'd also prefer a decentralized solution.. bitUSD, maybe niButs, nuBits maybe Schilling in Ethereum (once released)

If you are based in Canada or Australia, you can also check out a new site by quickbt.com , which is spendbt.com . I have bought from quickbt a couple of times, and they are trustworthy .

EDIT: I have never sold yet on spendbt, and just saw they haven't activated in Australia yet, even though quickbt is. And currently they seem to only sending the money to - Canadian Visa Credit Cards & Canadian Reloadable Visa Prepaid Cards

As far as i know they only do wired transfers.
What about trying localbitcoins.com

The only online casino on which i won something. I made 17mBTC from 1mBTC in like 15 minutes.  This is not paid AD!

Thanks guys.  Sounds like it's just Coinbase and Circle then.  I miss Buttercoin already.
