Once a transaction has 6 confirmations, it is extremely unlikely that an attacker without at least 50% of the network's computation power would be able to reverse it.

Grab a MoneyPak, get on to Tor, set up Tor SASL, goto #bitcoin-otc on freenode IRC, make your offer. Wait until someone responds, provide MP to a trusted trader there, receive BTC.  This will probably be the cheapest and safest routes for you to maintain anonymity.
I thought that you couldn't connect to freenode via tor until you register with nickserv.  Which means you need to connect without tor once to register.  So the admins could tell your IP from your registration if they wanted to, right?

That is correct! But what if the OP registered at Walmart in a different town?
That would work.  Unless big brother is watching you through all those CCTVs Tongue

The problem is now our "simple" instructions for "fast, easy, and anonymous" require driving to another town and using a public computer Sad

I have the same question. Mt Gox requires a verified account

Or is this paranoia not required and do they keep your info safe?

I think, the easiest and reasonably anonymous way is Bitinstant (VPN)/walmart blue slip Moneygram (fake name, address, etc...), to any wallet you wish.
