For a website to be added to the Premium category it needs to have a good amount of traffic, to be online for at least 30 days, to not have/not be related to incentivized traffic, a beautiful design is always a plus.
Of course, a website can be denied even if fits all these requirements if we consider that has low-quality content/traffic.

The process usually takes less than 24 hours if the campaign is made during our working hours.
All pending campaigns have been reviewed hours ago.

same here the Slider ad from coinzilla is killing my hole page its not loading corretly when i put silde ad from coinzilla on!!!
but some days ago it was no problem !!!

Never be afraid to ask questions. If you are looking to place multiple banners of the same size on one page, you should create new banners each time. This way, you will be credited for impressions across multiple banners instead of just the one.
