Why wouldn't people get 10% exchange rate in their favor by exchanging btc to paypal, then buy gift cards at 10% off?

You'll take a huge risk converting to Paypal, especially if somebody offers you 10% under spot rate for it.  No conversion, no bullshit, just paying 5% off the price for pizza.

The 5% is taken after the best coupon possible is used.  So you'd still save money.  If you aren't interested, don't order.

I believe Papa John's is possible to do worldwide, while dominos is US only.  All that is needed is delivery info, no payment information.  You can put a fake name if you wish, though do so at your own risk.
