I possibly can fill this on monday if no one has filled it yet, and if you need it as long as you can provide a signed message from one of the addresses with the date, your username and the loan amount.

And you want it in BTC, right? I'm not sure, but the fees the lender is going to pay are quite high currently, so that would make up quite a large percentage of the total amount...
