North Korea will survive, the US will only attack the military and government institutions. China will help US, but with the agenda of annexing the northern part of North Korea while the southern part will be joined to South Korea.

North Korea may be flattened by the Americans in the initial few hours. But even if just 50% of the population survives, they will launch a powerful counter-attack against the American bases in Japan and South Korea (probably using nukes). And this will cause tens of millions of casualties on both the sides.

Of course not. But that's not the real question. The real question is if the United States could survive the consequences of an action like that. A lot of well armed countries, like China and Russia, may decide that the USA is a too big danger for this planet, and act accordingly.
