The easiest way is probably to use one of the existing on-blockchain platforms suited for "assets" or "ICO tokens". In this case, you create an "asset" on a blockchain and then it becomes available for trading.
Some examples are Ethereum (ERC20 tokens), NXT, NEM, Waves or even Ripple. You should, however, take a look at the regulation in your country, in some of them it is becoming stricter now.
On centralized, traditional exchanges like Poloniex or Bittrex, normally a fee is paid (about $1000 minimum for a decent exchange) to list your token. Some exchanges have special modalities for ICOs (I think YoBit was one of them).
