Its concerning reading about BTC Mining pools getting hacked.  I had used slush for a while in the past but I've heard he has been hacked a few times due to issues with his hosting provider.

So my point is how often does this happen and which Mining pools do you trust the most and why?  Anyone using multifactor authentication?
