You submitted it on day one when you registered for the bounty.  It's your KMD address.  When the dICO is over your coins will be sent.  Which bounty did you sign up for?

Why am I deleted from the signature campaign list? I've been on the campaign for 9 months, and after you made the announcement that you can now remove the signatures, I removed the signature. please correct this error.  Signature campaign spreadsheets line: 10

Now that we have been told we can remove our signatures and avatars because the bounty is paused, will we have to add the avatar and signatures back once it begins again?  I don't want to get in with another campaign only to be forced to leave it.

I think this will only apply when the campaign starts up again. Then again, they may not require the start again. Its kind of up in the air - but I feel the ICO will be coming shortly!
