The tokens are not immediate distribution.
The team acknowledge the bounty hunter effort and create the form so that no one is left out. So do be patient and wait a while more for the distribution. A lot of bounty took months to finish ico and give out the tokens.
So, i still didn't get tokens, my last message was about 2 weeks ago, what is the reason of such situation for now?

Ofcourse i filled the form, and i send messages to admins in telegram about luckydraw and twitter compaign reward but as we can see, they dont care of such problems
so i guess LinkEye was never going to pay for this bounty. I have waited for months and nothing is forthcoming.

Hi, sorry guys but i have asked about the google form. I shall be honest about the situation.
The google form created by the coordinator yet to pass the form to the dev team.
However thanks to the highlight from the members here, the form was passed to the dev team 3 days ago. There was only 5 entries.
So sorry for all the trouble. Although I have not much involvement in this bounty, I still wish all the participants get what they deserve. My power is very limited for this bounty but i will try to support you all whatever i can.
