It is very common. You may want to audit your account (Balances->Audit My Accounts)

When the servers get hit hard, yes. I got buy/sell orders that I cancelled and got into negative..it all worked out since I made a profit off of those negative coins, but I would make sure to not be spamming the cancel/buy/sell button when this happens.
