I was trying to send payment from one address to two addresses. I confirmed that output #1 was correct but output #2 was unrecognised so I cancelled the whole transaction. It was showing in the transaction that output #2 displayed on the ledger nano S was the change address, but I got a little freaked out that the device was asking me to confirm an address which I did not input. Is this usual or is my electrum application compromised?

Your electrum application is not compromised.
This is a 'bug' in an older version of the nano s firmware. It asks to confirm the change output additionally to the first output.
You can get rid of this with updating your nano s.
  • Current Secure element version: 1.3.1
  • Bitcoin chrome app: 1.9.12
Make sure to have a backup of your 24 word seed phrase available in case of the need of reentering your phrase.
