How to Avoid Bitcoin and ether premium in some countries?

Hey guys I am trying to figure out if there is any way to avoid the Bitcoin premium in some exchanges in many of countries.For example in some countries Bitcoin and ether trade at a premium of 20 %..This means that people lose 20 percent of coins they want to buy since the btc or eth are needed to buy altcoins .

Is there any way to avoid these premiums? Are there any sites which allow to buy using PayPal or debit card so we can escape this ?
