These are never going to be natural. They will add new members from other active groups to your group through a few bot accounts that they operate on use & throw basis.
Back in 2017, these kinds of services were rampantly making money from shit ICO's that wanted to show a bloated telegram membership to fake their importance by the "community"...
I'd stay away from such services. I really find it annoying when I find myself added to new groups without every wanting to do so in the first place!
Yup thats my concern. Twitter is known to ban fake follower accounts too along with groups which they follow as well if its a majority of the followers. Can imagine Telegram following same suit.

Members will be real but engagement depends on what you post on your group.
I just don't see how will you get real users to follow a channel they may not relate to at all.
