I am assuming that this is remote work and there really is no need for you to know the city of residence instead you should ask for country and country only. Also the full name should be desired name and not requiring their legal name. You haven't given enough information on what the pay structure will be and whether or not it will be in Bitcoin.

It seems like you are gathering the personal information of the users here, since for me its not good to fill a form where we are not sure if we can get the job. Better to have an interview first and only give this form to your potential workers if they successfully chosen. Better contact people in private than requiring then to fill-up a form that they even don't know what project would be promoted/managed. Since this is an anonymous forum, never expect legitimate answer especially you as the hiring party didn't even mind to disclose your own information. It isn't really wrong to require personal info, it isn't just that encouraged especially with the anonymity of the most projects. In the end, people providing you their personal info can be used as your tool once you scammed people and point everything to those who submitted personal info right?

Too many questions the person is required to fill in while the minimum information about your project or company, conditions of working process and so on. Normally in such situations to point the Telegram or other social network via to contact with you or with other manager that is working with potential job-seekers of work.
