I mean like ever, maybe while pissed/stoned once?

No I haven't. (and it is advised not to, ever) But then neither have I ever been pissed/stoned.
Does drink/drug abuse increase the likelihood of such a condition?

if drink/drug abuse doesnt do , what else can?
