It's a powercolor 5850, exactly like the one show here:  http://www.newegg.com/Product/Product.aspx?Item=N82E16814131386

With one major exception... one of the blades on the fan is broke off, causing the fan to vibrate and make a lot of noise (and also not cool as well as it could).  The card otherwise works without issue.

Make me an offer on the card, or sell me a fan.  No reasonable offer will be rejected.  I'll accept btc or paypal.

Creating a Bitcoin client that fully implements the network protocol is extremely difficult. Bitcoin-Qt is the only known safe implementation of a full node. Some other projects attempt to compete, but it is not recommended to use such software for anything serious. (Lightweight clients like Electrum and MultiBit are OK.)

Have you checked out ebay? There are quite a few replacement fans out there. I found some for my 5870s.

If you don't have or don't want to use paypal give me a PM and I can act as a buying proxy for you (I did this for someone on the #bitcoin IRC chan a while back without any issues).
