Cavirtex, and canadian bitcoins, both of which I have tried and been happy with.  Canadian Bitcoins is pricey but VERY fast..

I recommend trying spendbitcoins.com; been pretty reliable for me in terms of payment and I can get money out via payapl (which can be good / bad for some). *Note: they only pay out in Paypal USD funds btw.

you said cash, do you mean a paypal transfer or a bank wire transfer. cause i think it'd be hard to acctually send you cash. maybe western union?

cavirtex will give you bank transfers...

I have done quite a lot of business with them recently(~10k). I loved using hyperwallet, but now only way is direct withdrawals which take 1-2 business days..

and then there's canadianbitcoins.com

they are awesome, while 30-50 cents BELOW cavirtex's rates(as of now, its 4.87 vs 4.49), but you get cash xpress posted to you.. so far turn around has been 24 hrs during the week.
I have cashed out 998.2 coins for $4425. super reliable. Their customer service is what made me choose to go with them, even if I am losing 500$ per every 1000 coins i cash out.. its ANONYMOUS cash so I guess 12% in fee's is reasonable...
