Most likely an issue with the power side of the board or a bad solder joint. I have no experience with these boards just electronics in general.

Even if you use Bitcoin through Tor, the way transactions are handled by the network makes anonymity difficult to achieve. Do not expect your transactions to be anonymous unless you really know what you're doing.

Got a dead hashboard on a S9 here as well... Posting a shot in case someone has a diagnostic for what is shows. Occured after a cold boot, was working fine before the shutdown.

Let it run till it gets hot then try a restart. Sometimes the boards flunk while cold, indicitive on bad solder joints between board and chip.
