What is the earliest block with a spend? Wouldn't Satoshi have wanted to have a real-world test of the spending mechanism?

You can't win if you don't play. But you can't play if you lose all your chips.

It is at least 100 blocks in because generates are not 'confirmed' until then. And the standard client doesn't let you spend them for 120 blocks. I bet he played with a private testnet first though so he probably wasn't in a hurry to test that.
