DO NOT FORGET THE LAMINATE! My wife spilled (rather, the soil of the plant-holder leaked) water on a password unlocking an encrypted hidden drive with scans of both of our tax documents on it, among other important docs. Of course, when I was hatching the plan to put the p/w on paper, I was thinking "Oh man, I'm so freakin' clever... I put 10 random characters on the paper, and between each letter goes a password I have memorized... even if someone finds it, they'll never be able to figure it out." When the water spilled, the ink became unreadable. I use this one. Syncs over browsers (auto-fill also) and you can also easy access your 'vault' trough their website. For program passwords, I use my memory and a notepad. :P
