Polish-English translation needed

hi,

i was working for a Polish registered company few months back, and ended short handed, since they ended up not paying.
i contacted "inspektor pracy" as soon as the deadline passed for paycheck, and left Germany (their work location).

I need a small documents translated to english (or croatian), since google translate is pure sh*t.
It is a respond i got from inspektor pracy today, its small and probably will take you like a minute to translate it correctly.

*please only good Polish speaking ppl, its important.

To be honest, if you are offering only $1 for translation of some "important & official papers" I don't think many people here would be interested.
Only people who have a lot of time on their hands and like to help people for free could appreciate your offer. I could help you, but again, for $1 I won't bother.
