If all nuclear weapons were unleashed at once it would be doomsday, but I don't think any government has things set up to do that. Full-scale nuclear war would consist of dozens or perhaps hundreds of nuclear explosions at certain strategic points, but an entire country would not be blanketed with explosions. This would result in short-term (a few months) danger from fallout and long-term (decades) economic damage due to "nuclear winter" climate change and illness/death due to the radiation. But we would not need to rebuild civilization from scratch or anything. I used to kind of hope for a nuclear war because I thought the shakeup might cause positive social change, but I now tend to think that it'd be bad for voluntarism. Probably it would lead to less rational thinking among the populace (due to panic) and a more violent government.

Perhaps not sticks and stones, but in the near-term after full-scale nuclear war, ammunition will be worth it's weight in gold. We'd need to rebuild the industrial base, probably a good chunk of infrastructure, and the social structure would be at best deeply shaken.
